RESIDENT LOG

 

 

Each resident shall maintain a log of devices they have become credentialed to measure, fit and adjust. It is the resident responsibility to keep the list up to date, and to inform their mentor when they feel they are ready to obtain the credential for a particular device. This list will be reviewed with the resident director during regularly scheduled meetings 

 

 

Instructions for completing credentialing form

 

·        Complete top form with Name and Clinic - for resident rotating through various clinics, note the clinic you were credentialed for a particular device

·        Keep the number of patients observed, assisted and fit for each category.

·        When you feel competent and comfortable delivering this particular level of care, discuss with your mentor, and request to be credentialed

·        All Charts still need to be signed by a certified practitioner.
